beckett hand signingPeople who enrol with Greenwood Health are entitled to GP consultations at a reduced rate. They may also be eligible for a range of free health services such as childhood immunisations, diabetic annual reviews, sexual health advice and treatment, counselling services, cervical smears, mammograms, heart disease risk assessments, smoking cessation services and palliative care. Other subsidies may be available for patients with more than one health problem who need regular reviews.

You can only be enrolled with one practice at a time.

Enrolment involves filling out a form with your details on it, agreeing to our Terms of Trade (which are "payment on the day" unless otherwise agreed) and authorising us to get any old notes from your previous doctor. An enrolment form must be filled out for each new patient and signed by someone over the age of 16 years.

Please also complete our Health Questionnaire -this enables us to quickly identify any potential issues that may need extra care. 

Please note that we are now required to see proof of your identification details - here is a list of the documents you will need to bring with you when you enrol at the surgery.
